If you have been involved in a crash:
All crashes involving property damage of greater the $500 or injuries must be reported to the police immediately. The police officer who investigates the crash will provide you with the report number you will need to get the driver exchange information from the Round Lake Park Police Department Records Department.
The Driver Exchange Report Form contains the following information:

Date, time, location & the crash report number.

Name, address, telephone number and insurance policy number of the other driver(s) involved.
What should you do next?

Contact your insurance agent as soon as possible - there may be a time period restriction for you to make your claim.

Have the report number available for him/her as they will need it to get a copy of the report. (This will help speed up your claim).

If damage to any vehicle in the crash exceeds $500.00 (estimated) or if there is an injury, each driver is required by State Law to complete the Motorist Report form and mail within 10 days to:

Information about your crash report:
- There may be blank boxes on your report form. These are for internal use or do not apply to your crash.
- Police officers are not required to complete additional reports on all crash reports.
- There will be a $5 fee for each copy of a crash report. Your insurance company will usually pay the fee when requesting a copy of the report for claims purposes.
What about tickets issued by the police officer?
- If the other driver receives a traffic ticket:
You have to appear in court unless notified by the officer otherwise.
- If you receive a traffic ticket:
Two copies of a ticket are issued:
The Violator's Copy is yellow. This explains the violation and is used as receipt for bond (i.e. driver's license, bond card, cash posted for bond). Instructions on how to handle are listed on the back.
The Court Communication Copy is blue. This must be used to make payment or to request a trial date.
There are basically three options when you receive a traffic ticket:
- Plead Guilty - Send the required fine to the court via mail.
- Plead Not Guilty - Request a trial date using the court.
- Traffic School/Court Supervision - Complete traffic school requirements (Non-Injury accidents only).
Traffic tickets are not issued for crash occurring on private property, including parking lots. (Except for D.U.I. or Reckless Driving.)
Where do you get copies of the crash report?
Copies of completed reports are available from the Round Lake Park Police Department at 215 Main Street approximately 8-10 business days after the crash.
